To file a claim with Transafrica Life Lesotho, policyholders or beneficiaries should:
- Complete the claim form in black ink and block letters.
- Submit the completed form along with the following supporting documents:

- Certified ID copy of the claimant (either main member or beneficiary).
- Certified ID copy of the deceased with “deceased” stamped on it.
- Certified copy of the death certificate.
- Copy of the BI-1663 form.
- Copy of the Burial Order.
- Bank statement with a bank stamp of the claimant or beneficiary on the policy.
- Police report in cases of death due to unnatural causes.
- Any other documents that Trans-Africa Life may, at its discretion, require.
Trans-Africa Life Lesotho will contact you once your claim has been assessed. All deaths will be verified with the Department of Home Affairs. Depending on the circumstances, there may be other requirements. Please ensure that you meet all the requirements set out in the claim form.
